We bridge the gap between hospital and home for medically complex patients by combining state-of-the-art treatments with a team of highly skilled nurses , therapists and on-site physicians in a hospital setting.

Our programs and treatments are the next step in the continuity of care and are designed to restore and strengthen patients so they can return to their highest level of independence. Our interdisciplinary team approach to care allows us to treat a wide variety of medical conditions. We meet with our patients about their progress and provide family support, education and training about what is happening to you or your family member.

Anyone who is limited functionally from an injury or illness can benefit from rehabilitation. A patient’s stay at our hospital can range from several days up to several weeks, depending on medical condition and your treatment plan. Our level of rehabilitation care increases your independence and reduces healthcare costs.

We admit patients seven days a week. Referrals may be made by patients, physicians, hospital discharge planners, insurance representatives, employers, family, friends, private rehabilitation providers, case managers and allied health professionals.
